A new form for registered International Fuel Tax Agreement (IFTA) carriers who want to request additional IFTA decals and/or a Temporary IFTA Permit for a commercial vehicle that requires immediate authorization prior to receiving the additional IFTA decal set(s) has been created:

FIN365- Request for Additional IFTA Decals and Application for a Temporary IFTA Operating Permit
