  • Timely access to a spine surgeon and pain specialist is available if needed.
  • Referral to integrated pain, substance use, and mental health services are offered if required during low back pain care.

Who Can Participate?

Those experiencing persistent low back pain or related symptoms not improving within 6 weeks to 12 months, or individuals with unmanageable recurrent/episodic low back pain less than 12 months post-recurrence are eligible.
